Job Summary

The Care Coordinator is responsible for ensuring seamless transitions for consumers within the agency and community, providing linkages to physical and behavioral health, community services, and other needed services to facilitate wellness and recovery.

This role requires a consumer-focused, detail-oriented, and well-organized individual with proficiency in managing electronic health records and experience with community resources, public health, and special populations.

Key Responsibilities
  • Ensure linkages for CCBHC consumers within the agency and community
  • Provide seamless transitions across the health services spectrum
  • Facilitate wellness and recovery of the whole person based on a person-centered plan of care
  • Manage multiple priorities and work with multi-disciplinary teams
  • Document care coordination in the electronic health record accurately and timely
Requirements
  • Minimum associate degree or diploma in nursing
  • Professional licenses in Kentucky required
  • Three years' experience in nursing preferred
  • Experience working with community resources, public health, and special populations preferred
Compensation

$35,000 - $39,000
