Homecare Physical Therapist

Intermountain Healthcare is seeking a skilled Physical Therapist to join our Homecare team. As a Physical Therapist, you will provide quality physical therapy services to patients and families in the comfort of their own homes.
Job Summary:This is a part-time, non-exempt position that requires a current Physical Therapy License in the state of practice. The ideal candidate will have excellent communication skills, basic computer skills, and the ability to work independently.
Responsibilities:- Provides competent theoretical and technical skills pertinent to the patient populations being served.
- Develops and implements appropriate care plans and goals.
- Coordinates patient care with other disciplines and the case manager.
- Provides timely and thorough documentation of the patient record consistent with agency standards.
- Efficiently uses resources by effectively managing time, supplies, and equipment.
- Current Physical Therapy License in the state of practice.
- Basic life support certification (BLS) for healthcare providers.
- Access to the Internet or a phone line transferring patient files twice a day.
- Demonstrated effective communication skills.
- Basic computer skills.
