Job Description

The ideal candidate will have experience in home health care, with a strong understanding of nursing assessments and patient management. This role will involve conducting physical assessments, overseeing caregiver teams, and ensuring effective case management to meet clients' health needs.

Responsibilities
  1. Conduct regular physical assessments, including vital signs and home safety evaluations.
  2. Oversee skilled and non-skilled caregivers providing care in the client's home.
  3. Develop and implement individualized care plans tailored to each client's needs.
  4. Collaborate with regional directors and other personnel to identify and address any issues or areas for improvement.
  5. Participate in client care conferences as needed and serve as a local on-site clinical resource.
  6. Adhere to nursing delegation guidelines and ensure adherence to Agency policies.
Requirements
  • Graduate of a state-approved school of professional registered nursing.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two years of nursing experience, including one year in home care or a closely related field.
  • One year of supervisory and/or case management experience preferred.
  • Current CPR certification.
