Job Summary

Bridgewell is seeking a Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will assist in coordinating medical services and providing healthcare support to individuals with disabilities.

Key Responsibilities
  • Assist in coordinating medical services with primary care physicians, medical clinics, and residences
  • Provide healthcare services to individuals, including medication administration and medical documentation
  • Establish and oversee groups in relevant areas, such as hygiene and women's or men's groups
  • Provide in-services and training to staff on pertinent medical information
  • Inform the clinical team of medical changes, needs, and concerns
Requirements
  • Current registration with the Massachusetts Board of Registration on Nursing
  • One year of experience working with individuals with disabilities
  • One year of experience in a community setting
  • Knowledge of Day Habilitation Regulations and CARF standards
