We are seeking a skilled Clinical Care Provider to join our team at Salud Family Health in Longmont, CO. As a Clinical Care Provider, you will be responsible for providing high-quality patient care in a team-based care fashion that supports patients and their families in self-management, self-efficacy, and behavior change.

Key responsibilities include performing routine clinic operations, taking histories, and making physical assessments. You will also be expected to adhere to the spirit of the SALUD mission statement.

Requirements:
  • Must have graduated or been graduating from an accredited NP or PA program

Licensure:

  • Possess an unrestricted PA or NP license in good standing in the State of Colorado

Knowledge, Skills, and Abilities:

  • The ability to speak Spanish is strongly preferred
  • Sensitivity to low-income and ethnic minority communities is a must

Experience:

  • 3 years of experience in a clinical setting as a Nurse Practitioner / Physician's Assistant preferred
