As a key member of our interdisciplinary team, you will play a vital role in promoting the physical, mental, and psychosocial well-being of our patients/residents.

Key Responsibilities
  • Collaborate with the interdisciplinary team to identify and address the social, emotional, and mental health needs of patients/residents.
  • Develop and implement individualized plans of care to meet the unique needs of each patient/resident.
  • Provide education and support to patients/residents, families, and significant others to improve quality of life.
  • Coordinate discharge planning activities, including referrals to community-based agencies and services.
  • Establish and maintain relationships with community-based agencies and services to benefit our patient/resident population.
