Overview

AccentCare is seeking a skilled Licensed Vocational Nurse (LVN) to join our Home Health team on a weekend schedule. As a LVN, you will provide high-quality patient care and support to patients in their homes, working closely with our interdisciplinary team to deliver comprehensive care.

Key Responsibilities
  • Provide nursing visits to patients in their homes, following treatment plans prescribed by physicians
  • Document patient information and medical records accurately and thoroughly
  • Collaborate with the healthcare team to ensure seamless care coordination
  • Communicate effectively with patients, families, and healthcare professionals
Requirements
  • One year of clinical experience and one year of home health experience
  • Ability to provide nursing visits according to treatment plans and maintain accurate medical records
  • Currently licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
  • Valid CPR certification
  • Licensed driver with ability to travel to all business locations
