Job Title: Clinical Pharmacist
Job Summary:
We are seeking a highly skilled Clinical Pharmacist to join our team at the University of Virginia. As a Clinical Pharmacist, you will play a critical role in ensuring the safe and effective use of medications in our medical center.
Key Responsibilities:
- Medication Order Verification: Ensure that medication orders are accurate, complete, and safe for patients.
- Drug Therapy Monitoring: Continuously monitor inpatient and outpatient drug therapies to prevent adverse reactions and optimize patient outcomes.
- Collaboration with Medical Staff: Work closely with healthcare providers to find alternative treatment options and ensure the best possible care for patients.
- Clinical Pharmaceutical Care: Provide comprehensive pharmaceutical care to patients, including medication counseling and education.
- Quality Improvement: Enhance the quality of care delivery through process improvements and professional development.
- Education and Development: Support the education and professional development of our pharmacy team.
- Regulatory Compliance: Ensure compliance with regulations and standards related to medication use and management.
Requirements:
- Education: Bachelor's degree in Pharmacy or PharmD required
- Experience: PGY-1 residency required or equivalent hospital experience (3 years). PGY-2 residency preferred for specialty areas.
- Licensure: Commonwealth of Virginia State Board of Pharmacy license required. American Heart Association (AHA) Health Care Provider BLS certification required.
