Job Summary:
We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our team of dedicated caregivers. As an LVN, you will provide high-quality patient care in a home-like setting, working closely with our clients and their families to ensure their physical, emotional, and social needs are met.
Responsibilities:
- Provide one-on-one care to clients, following their individualized care plans and promoting their dignity and independence.
- Monitor clients' vital signs and report any abnormal findings to the case manager and physician.
- Administer medications, feedings, oxygen, and other treatments as needed, and document the results.
- Complete routines established by other care professionals, such as physical therapists, occupational therapists, and speech therapists.
- Ensure infection control policies are maintained at all times, reporting any possible communicable diseases or infections.
- Accurately document nursing actions and communicate with family members, case managers, and physicians.
- Order supplies, medications, and feedings as needed to ensure adequate stock levels.
- Monitor incidents involving clients, ensuring incident reports are completed timely and accurately.
- Ensure all medical equipment is maintained and report any faulty equipment to the Durable Medical Equipment department.
Qualifications:
- Current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- Current Cardiopulmonary resuscitation certification.
- Ability to work in a home-like setting.
- Physical ability to bend, stoop, push, pull, sit, and walk for periods of time.
- Able to utilize proper body mechanics while lifting 50 pounds.
- Knowledge of individual service plans, including understanding of client rights and health needs.
