At Interim HealthCare of Sacramento, we are seeking a dedicated and compassionate Registered Nurse to join our Hospice team as a Case Manager RN. As a key member of our interdisciplinary care team, you will play a vital role in providing end-of-life care to patients and their families, ensuring their physical, emotional, and spiritual needs are met with dignity and respect.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' needs to develop individualized care plans.
  • Coordinate care team meetings to ensure collaboration among healthcare professionals.
  • Educate patients and families about disease progression, symptom management, and end-of-life care options.
  • Maintain accurate documentation of patient assessments, care plans, and outcomes.
  • Provide compassionate end-of-life care, including symptom management and support for patients and families.

Requirements:

  • Graduate of an accredited nursing program and active RN license in California.
  • Minimum of 1 year of nursing experience in hospice.
  • CPR certification.
  • Knowledge of state and federal home health regulations.
