Job Description

At Mass General Brigham Home Care, we are seeking a skilled Home Health Nurse Specialist to join our team.

About the Role:

  • We offer a comprehensive compensation package, including a competitive $80,000 - $120,000 per year salary range, based on experience.
  • The selected candidate will be responsible for providing direct patient care in a home setting, assessing patients' health status, developing and implementing individualized care plans, and collaborating with physicians and other healthcare professionals.
  • Key Responsibilities:

  1. Conduct thorough initial and ongoing assessments of patients' physical, emotional, and social factors, identifying areas of strength and need for intervention.
  2. Develop and implement individualized care plans in collaboration with the patient's physician, outlining nursing interventions, goals, and expected outcomes, ensuring seamless transitions between healthcare settings.
  3. Educate patients and their families about their medical conditions, treatment plans, medications, and self-care techniques, promoting wellness and independence.
  4. Monitor patients' progress and response to treatment, modifying care plans as needed to ensure optimal health outcomes.
  5. Coordinate care with other healthcare providers, community resources, and support services to provide continuity of care.
  6. Collaborate with multidisciplinary teams to develop and implement effective care strategies, utilizing evidence-based practices and research to improve patient outcomes.

Requirements:

  • Licensure: Active RN licensure in Massachusetts
  • Experience: 1+ years of home health or hospice nursing experience
  • Certifications: BLS, preferably ACLS and PALS
