Clinical Pharmacist

We are seeking a highly skilled Clinical Staff Pharmacist to join our team at Methodist Hospital-Southlake in Merrillville, Indiana. As a Clinical Staff Pharmacist with CPS Solutions, LLC, you will play a critical role in ensuring the safe and effective use of medications in our hospital setting.
Key Responsibilities- Evaluate and interpret orders, compound, and dispense medications according to written orders/prescriptions issued by authorized prescribers.
- Identify, prevent, assess, and resolve medication-related problems to ensure a safe and optimal patient outcome.
- Promote clinically rational drug therapy and sound pharmaceutical care through criteria-based protocols.
- Work collaboratively with healthcare teams to provide high-quality patient care.
- Pharmacy degree (BS or PharmD) from an ACPE accredited school of pharmacy; PharmD preferred.
- Current Indiana Pharmacist license in good standing.
- Successful completion of a Pharmacy Practice Residency program strongly preferred.
- At least 1 year of recent work experience as a Pharmacist (hospital preferred).
- Ability to work independently, set priorities, make critical decisions, and respond to client concerns.
- Demonstrated accuracy and timeliness in a work setting.
- Experience using computer applications including word processing, spreadsheets, and pharmacy information systems.
- Effective communications skills; verbal and written.
