Key Responsibilities:

  • Adhere to specific care plans for clients and document completed tasks
  • Assist clients with personal care requirements (bathing, dressing, etc.)
  • Provide or assist with routine household cleaning, meal preparation, and laundry
  • Transport clients to essential appointments and errands
  • Support clients in self-administering medications
  • Monitor and report any changes in the client's condition
  • Maintain confidentiality due to access to sensitive information
  • Exhibit consistent attendance and flexibility to meet departmental needs
  • Comply with all Medicare, Medicaid, and HIPAA regulations
  • Follow organizational policies, procedures, and standards
  • Perform other assigned duties as necessary

Position Requirements & Competencies:

  • Must be at least 18 years of age
  • Must be a Certified Home Health or Personal Care Aide
  • Pre-employment physical examination/PPD required prior to patient contact
  • Must pass a criminal background check
  • Must have reliable transportation and a valid driver's license with insurance
  • Compassionate and nurturing disposition with a commitment to caring for others
  • Ability to work independently with minimal supervision
  • Ability to follow both written and verbal instructions
  • Strong communication and interpersonal skills
