We are currently seeking a skilled Licensed Vocational Nurse (LVN) / LPN to join our team in a challenging and rewarding role as a Sub-Acute Care Specialist. This position is an excellent opportunity for experienced professionals to showcase their skills and dedication to patient care.

In this role, you will work in a dynamic environment where no two days are the same. Our facility, with 536 total staff beds, offers a unique blend of medical and therapeutic services. As an LVN/LPN, you will play a vital role in delivering high-quality patient care, working closely with interdisciplinary teams to achieve exceptional outcomes.

Key Responsibilities:

  • Deliver comprehensive nursing care to patients in a sub-acute setting, prioritizing their physical and emotional well-being
  • Collaborate with healthcare professionals to develop and implement individualized care plans
  • Monitor patient progress, making adjustments to care plans as needed
  • Conduct thorough assessments, documenting observations and interventions in patient records
  • Participate in educational programs to enhance your skills and stay up-to-date on best practices

Requirements:

  • 1 year of experience as an LVN/LPN in a similar setting
  • Current licensure to practice as an LVN/LPN in the state of employment
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
  • Strong communication and interpersonal skills, with the ability to work collaboratively with colleagues and interdisciplinary teams
