Job Summary:
Cynet Health is seeking a skilled Traveling Registered Nurse - Case Manager to join our team. As a Traveling Registered Nurse - Case Manager, you will be responsible for coordinating patient care and providing case management services to patients in need.
Responsibilities:
- Participate in family meetings and planning to ensure comprehensive care for patients.
- Coordinate discharge planning with a focus on care coordination to ensure smooth transitions for patients.
- Utilize InterQual for admission criteria to ensure accurate and efficient patient placement.
- Provide options for drug and alcohol abuse treatment to support patients in need.
- Work with patients experiencing homelessness to provide comprehensive care and support.
- Manage and support patients with a mental health history or in crisis to ensure their safety and well-being.
- Understand and work with Advance Directives and POLST to ensure patient autonomy and dignity.
- Coordinate care for adoption and foster care cases to ensure the best possible outcomes for patients.
- Arrange DME, SNF, and ambulance transport efficiently and independently to ensure timely and effective patient care.
- Engage in the development of treatment plans to ensure comprehensive and individualized care for patients.
- Navigate and facilitate protective services processes to ensure patient safety and well-being.
- Comply with documentation and reporting requirements to ensure accurate and timely patient information.
- Will be required to work three of the five specified holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day.
- Maintain communication devices as required by the assignment to ensure effective communication with patients and colleagues.
- All nurses are expected to float between facilities as needed to ensure adequate staffing and patient care.
Requirements:
- Travelers must have references covering one year of employment within the last three years.
- Floating between facilities within a state may be required, with refusal potentially resulting in assignment termination.
