About the Role

Virginia Mason Franciscan Health Rehabilitation Hospital is seeking a skilled Case Manager to join our interdisciplinary team. As a Case Manager, you will play a vital role in the care and recovery of patients in our 60-bed state-of-the-art, inpatient acute rehabilitation hospital.

Responsibilities
  • Collaborate with the patient care team to develop and implement individualized treatment plans
  • Coordinate weekly patient care team conferences to facilitate development, monitoring, and refinement of treatment plans
  • Communicate with patients and caregivers to ensure informed decision-making and progress updates
  • Review patient records to identify potential missed comorbid conditions and communicate findings to the HIM team
  • Perform other duties as assigned
Requirements
  • Masters or Bachelors in Social Work or as required by state
  • Certification in Case Management or Rehabilitation Nursing preferred
  • Minimum of 2 years social work or case management experience in an inpatient setting
  • Effective oral and written communication skills in English
  • Basic computer skills in excel, word, outlook, power point, etc.
