Key Responsibilities
  • Conduct thorough clinical assessments and develop individualized plans of care in accordance with federal and state program requirements.
  • Ensure patient eligibility and medical necessity for services, collaborating with other community providers as needed.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with the care team to revise plans of care as needed, ensuring patient needs are met and care is delivered in a timely and effective manner.
Requirements
  • Current RN licensure in the state of practice.
  • Minimum of one year experience as an RN, with a strong background in home health or private duty nursing.
  • Current CPR certification and reliable transportation.
