Nurse Care Manager

The Hospital Care Manager plays a vital role in ensuring seamless patient care transitions from acute care settings. This position requires strong collaboration with physicians and multidisciplinary teams to deliver high-quality, cost-effective care.
Key Responsibilities- Partner with attending providers to promote efficient utilization of clinical resources and ensure quality care.
- Anticipate next steps and facilitate communication to maximize care efficiencies for patients and families.
- Identify social determinants of health that increase patient risk for negative outcomes and develop strategies to mitigate these risks.
- Ensure clear documentation for the interdisciplinary care team and coordinate post-acute plans with the ambulatory care management team as needed.
- Facilitate patient access to resources and relevant services.
- Address and resolve system problems impeding diagnostic or treatment progress.
- Navigate value-based care with expertise and ensure longitudinal plans align with patient and caregiver goals.
- Assess patients' and families' psychosocial risk factors through evaluation of prior functioning levels, support systems, and reaction to illness.
- Maintain expert-level knowledge of body systems and expected clinical outcomes for patient disease processes.
- Ensure medical necessity, appropriate level of care, and timely implementation of plans of care in accordance with hospital Utilization Review Plans.
- Navigate complex compliance, regulatory, and insurance requirements, coordinating with third-party payors as needed.
- Bachelor's degree in nursing
- Three years of acute care nursing experience, with one year of acute care/case management experience preferred
- Licensed to practice as a Registered Nurse in South Carolina
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred
