We are seeking a compassionate and skilled Registered Nurse to join our Hospice team at WVUHS Home Care, LLC. As a Hospice RN, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.
Key Responsibilities:- Provide direct and indirect nursing care to patients in their homes
- Collaborate with the patient, family, and healthcare team to develop and implement individualized care plans
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
- Administer medications and treatments as ordered by physicians
- Monitor patient progress and adjust care plans as needed
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
- Document patient information and care activities in a timely and accurate manner
- Current licensure as a Registered Nurse in the state of West Virginia or ability to obtain licensure
- Valid driver's license
- CPR certification
- One year of experience in home care or hospice nursing
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Ability to lift, move, and position patients of all weights
