Job Summary

This position provides medical care for residents to ensure the highest degree of quality resident care is maintained at all times within a safe environment. The Licensed Practical Nurse will assist with activities of daily living, offer emotional and social support, and attend to residents' overall physical comfort and well-being.

Essential Functions
  • Assists and participates in the development of total care plans for residents with physicians, psychiatrists, nurses, and allied therapists.
  • Provides nursing care for residents and assists in rehab programs and bowel and bladder training programs.
  • Observes and reports on residents' physical and mental condition, obtains vital signs, and keeps patient records and reports according to policy.
  • Provides leadership and directs the functions of Certified Nursing Assistants.
  • Assists with admissions, discharges, transfers, and appointment scheduling.
  • Notifies the charge nurse of any change in patient condition and investigates and reports any allegations of abuse or caregiver misconduct.
  • Administers medications and treatments as prescribed by physicians and psychiatrists.
  • Contacts medical providers and families of residents related to changes in condition or concerns.
  • Receives MD orders, makes rounds with MDs, transcribes orders, and communicates orders to nursing staff and pharmacy.
  • Assists in the care and monitoring of IVs per facility policy.
  • Monitors and assists residents with psycho-social events.
  • Provides support, information, or education to residents or family members as needed.
Requirements
  • High School Diploma/equivalent.
  • Completion of a practical nursing program.
  • Wisconsin Practical Nurse License.
  • Previous experience as a CNA or LPN preferred.
  • Must successfully pass a criminal and caregiver background check.
Work Environment

This work requires the occasional exertion of up to 75 pounds of force. Work regularly requires sitting, frequently requires standing, speaking or hearing, using hands to finger, handle or feel, and repetitive motions. Work has standard vision requirements. Vocal communication is required for expressing or exchanging ideas by means of the spoken word. Hearing is required to perceive information at normal spoken word levels.
