Key Responsibilities:- Coordinate case management of all patient services, ensuring timely and effective delivery of care.
- Supervise assigned care staff, providing guidance and support to ensure high-quality patient care.
- Provide direct patient care as necessary, including case management, Oasis care planning, and assigning care staff.
- Ensure the efficient and effective utilization of existing staffing resources.
- Current RN licensure in North Carolina.
- Minimum three years of clinical nursing experience, with a focus on hospice care.
- In-depth knowledge of hospice/homecare regulations.
- Ability to independently organize work, provide case management, and timely and accurate documentation of care.
- Strong critical thinking and communication skills.
- Ability to use a personal computer and have a working knowledge of various computer applications.
- CPR certification.
