Job Description
We're seeking a skilled Home Health Care Professional to join our team. In this role, you'll provide one-on-one patient care, conduct thorough assessments, and develop individualized care plans to ensure our patients receive the best possible outcomes. Your compassion, attention to detail, and excellent communication skills will make you a valuable asset to our organization.
Required Skills and Qualifications
To be successful in this role, you'll need:
- a current RN license and certification in home health care
- at least 2 years of experience in home health care or a related field
- excellent assessment and critical thinking skills
- ability to work independently and as part of a team
