About the Role:
We are seeking a highly skilled and compassionate Clinical Educator to join our team at Good Samaritan. As a Clinical Educator, you will play a vital role in providing education and resources to healthcare professionals, promoting patient care and education, and collaborating with physicians and healthcare teams to develop care plans.
Key Responsibilities:
- Assess patient needs for self-care instructions in diabetes, wound care, ostomy care, bowel and bladder training
- Collaborate with healthcare teams to develop care plans for patients requiring education and/or intervention in diabetes, wound, ostomy, bowel and bladder care
- Provide education and resources to patients, significant others, and healthcare professionals to promote health and wellness
- Utilize age-specific criteria to prescribe appropriate actions to meet patient needs
- Document patient care and education in electronic medical records
- Maintain communication with intra and interdepartmental teams to ensure continuity of care and patient education
- Coordinate and manage department Performance Improvement initiatives
Requirements:
- Graduate of an accredited school of nursing
- Current Registered Nurse licensure in the state of Indiana
- Three years of nursing practice and teaching experience
- Bachelor's degree preferred in Nursing
- Wound Care certification highly preferred
- Computer skills including Microsoft, PowerPoint, Word, and Excel
