Responsibilities
  • Provide direct patient care as defined in the state Nurse Practice Act
  • Implement care plans initiated by the registered nurse
  • Document patient care accurately and timely
  • Assess and educate patients and families on self-care techniques and diagnosis-related information
  • Participate in coordination of home health services and report identified needs to the registered nurse and/or Clinical Supervisor
  • Use and prepare equipment and supplies effectively and efficiently
  • Participate in personal and professional growth and development
  • Perform other duties as assigned by the registered nurse
Qualifications
  • Graduate of an accredited practical nurse or vocational nursing program
  • Three years of nursing experience, with community/home health or medical/surgical experience preferred
  • Current licensure as a licensed practical nurse or licensed vocational nurse in the state
  • Valid CPR certification
  • Reliable transportation, current driver's license, and appropriate automobile insurance
