Our Home Health team is seeking a skilled Licensed Practical Nurse (LPN) to provide direct care and coordination to patients in their homes. As a key member of our interdisciplinary team, you will work closely with patients, families, and caregivers to ensure a seamless care experience. With a focus on clinical excellence and patient-centered care, you will have the opportunity to make a meaningful impact in the lives of our patients and their families.

Responsibilities:

* Provide direct care to patients in accordance with a comprehensive care plan in the home care setting
* Assess and provide patient and family/caregiver education
* Participate in coordination of home care services
* Ensure compliance with all state and federal legal and regulatory requirements

Qualifications:

* Graduate of an accredited practical or vocational nursing program with current licensure to practice in state of operation
* 1 year nursing experience required, 3+ years preferred
* Experience in community health, hospice or med/surg preferred
