We are seeking a highly skilled Nurse Practitioner to lead our School-Based Asthma Therapy (SBAT) Program. As a key member of our team, you will play a vital role in assessing, planning, implementing, and evaluating the SBAT Program.

Key Responsibilities:
  • Program Development and Implementation:
    • Develop and manage the implementation of the SBAT Program at new schools
    • Collaborate with school nurses and patient identification teams to ensure seamless program execution
    • Communicate with families and primary care providers to obtain necessary consent and authorization
    • Transport medications to schools and follow up with school nurses, families, and primary care providers regarding patient response to the program
  • Education and Training:
    • Act as a clinical resource for school nurses and families, providing education and support on asthma management and treatment
    • Assist in assessing the continuing education needs of the healthcare team and develop training programs to enhance knowledge and skills
    • Maintain clinical competency and stay informed of current trends in asthma care, participating in professional development activities to improve patient care and nursing practice
  • Patient Management and Medication Administration:
    • Function within the APN practice guidelines and in conjunction with the collaborating physician to develop and implement patient care plans
    • Prescribe medications within the Ohio Board of Nursing formulary and the CHI formulary, communicating with PCPs to manage child medications and reconcile home and school medications when necessary
  • Leadership and Collaboration:
    • Recruit new schools for participation in the SBAT Program, collaborating with school administrators and healthcare teams to ensure program success
    • Participate in education, research, and professional development activities to improve patient care and nursing practice, establishing professional objectives and evaluating self-progress
Requirements:
  • Master of Science Degree in Nursing or equivalent
  • Certificate of Authority to practice as an Advanced Practice Nurse in the State of Ohio
  • Certificate to prescribe
  • Valid Ohio Driver License
  • Minimum of three years nursing experience
  • Demonstrates nursing competence with specific clinical skills and knowledge in asthma disease management
  • Demonstrates leadership, interpersonal skills, teaching ability, advanced level of critical thinking and decision making
  • Excellent verbal and written communication skills
Physical Requirements:
  • Must be able to see, hear, stand, walk, speak, read, and perform manual tasks with or without accommodation
  • Must be able to independently lift up to 25lbs
