About the Role

Banner Health is seeking a skilled Physical Therapist to join our Home Care team in Greeley, Colorado. As a Physical Therapist in Home Care, you will have the opportunity to work with patients in their own homes, providing personalized care and rehabilitation services.

Key Responsibilities
  • Evaluate patients' functional abilities and limitations, and develop individualized treatment plans to address their needs.
  • Implement treatment plans using a variety of techniques and modalities, including physical therapy interventions and education.
  • Document patient progress and communicate with healthcare teams to ensure seamless care.
  • Work collaboratively with patients, families, and healthcare teams to achieve optimal patient outcomes.
Requirements
  • Graduate of an APTA-accredited school of Physical Therapy or equivalent.
  • Current Physical Therapy license in the State of practice.
  • BLS certification required.
  • Ability to become proficient with computer software applications commonly used in an automated patient care environment.
