Your expertise will be essential in developing and implementing individualized plans of care to optimize patient outcomes.
Key Responsibilities:- Provide direct occupational therapy services to patients in their homes, as directed by the occupational therapist and in accordance with the physician's plan of care.
- Communicate effectively with patients, family members, and caregivers to ensure seamless care coordination.
- Collaborate with other healthcare professionals to discuss patient plans of care and ensure comprehensive care.
- Plan for patient discharge, prepare discharge instructions, and communicate discharge plans with patients, family members, and caregivers.
- Participate in in-service programs to stay up-to-date on best practices and industry developments.
- Graduation from an Occupational Therapist Assistant education program from an accredited university/program.
- Current, unrestricted state license as an Occupational Therapy Assistant in the state of practice.
- Valid CPR certification.
