Key Responsibilities:
As a Primary Care Nurse Practitioner, you will work under the guidance of a licensed physician to deliver comprehensive healthcare services. Your role will involve collaboration with a multidisciplinary team to ensure high-quality, efficient patient care.
Core Duties:
- Conduct thorough evaluations and monitor patient health in partnership with physicians and other healthcare providers, adhering to established care standards.
- Assess the health status of new and existing patients through detailed medical histories and examinations.
- Analyze patient responses to current care and contribute to the development of individualized care plans.
- Maintain accurate and timely documentation of patient records and treatment plans.
- Prescribe medications within the scope of practice and regulatory guidelines.
- Collaborate with healthcare teams to enhance service delivery and patient care outcomes while upholding ethical standards.
- Advocate for patients by ensuring appropriate documentation and participating in insurance reviews to optimize treatment duration.
- May lead clinical coordination efforts and participate in special projects related to practice management and protocol development.
- Engage as a member of a multidisciplinary team, attending meetings to address clinical concerns and improve communication.
- Focus on achieving quality metrics through continuous improvement of patient care and operational efficiency.
- Adhere to departmental policies and procedures, ensuring compliance with safety and quality standards.
- Foster positive relationships with colleagues, patients, and visitors, demonstrating respect and empathy in all interactions.
- Enhance professional development through ongoing education and participation in training sessions.
- Evaluate personal performance and identify opportunities for professional growth.
- Participate in community education and outreach initiatives as needed.
- Bachelor's Degree in Nursing is required.
- Certification in CPR, Handle with Care, and Basic Life Support is mandatory.
- Current state licensure as a Nurse Practitioner is essential.
- Certification in Family Medicine or Internal Medicine is required.
- Completion of an accredited Nurse Practitioner program is necessary.
- Proficient understanding of nursing principles and their application in practice.
- Strong communication skills to effectively engage with patients and colleagues.
- Ability to build and maintain effective working relationships across various departments.
