We are seeking a compassionate and responsible LPN/RN to provide high-quality patient care in a home environment. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:
  • Provide one-to-one patient care in a safe and comfortable home environment.
  • Assess patients for stability and follow through with all doctor's orders.
  • Verify medication milligrams, last day dosed, take outs received, phase level, and verify guest doses.
  • Document patient complaints, vital signs, current medication, and allergies thoroughly and accurately.
  • Conduct health promotion, teaching, and training of family members as needed.
  • Perform other duties as assigned.
Requirements:
  • High school diploma or equivalent and graduate of an accredited program.
  • Must have a good standing license (LPN) in the state in which the clinician will practice.
  • At least 12 months of hands-on nursing experience.
  • Current CPR certification.
  • Willingness to provide in-home nursing care to infants, children, and adolescents.
  • Comfortable and efficient in the use of EMRs and computers.
  • Experience with GTube, Trach/Vent.
  • Detail-oriented with excellent interpersonal communication skills.
