About the Group

  • The group is a local primary care provider serving the greater Sussex County area and has been in business for over 25 years.
  • Physician Owned and Physician Led, offering partnership opportunities.
  • 4 Total Locations.
  • 23 Total employees.

Provider's Role with the Company

  • Location: Seaford, DE (50 minutes from Dover).
  • Outpatient primary care in the clinic setting.
  • Monday - Friday, 8:30am - 5:30pm, 100% outpatient.
  • Call: 1 in 6 weekend call, video or phone call only.
  • Estimated patient volume: 25 per day.
  • Patient age: 18+.
  • EMR: Cerner.
  • PA and NP support staff.

Responsibilities and Expectations

  1. Assess, maintain, and improve the health status of individuals in the communities served through timely access to evidence-based acute, preventive, and chronic disease care.
  2. Serve as a role model of professionalism to all members of the interdisciplinary healthcare team.
  3. Collaborate with all other practitioners and clinical support staff to form a high-performing team.
  4. Provide high-value care; demonstrate good stewardship of resources and consider risks, benefits, and patient preferences when making diagnostic or therapeutic intervention decisions.
  5. Document all patient care activities and services provided in the electronic health record. Documentation must be clear, concise, accurate, updated, and in sufficient detail.
