Job Summary:

We are seeking a skilled LPN Clinical Liaison to join our team at Heartland Hospice. This role involves providing ongoing information on our hospice services, coordinating referrals, and communicating effectively with patients, families, and referral sources.

Responsibilities:

Provide education about hospice, clinical outcomes, and benefits to referral sourcesMeet with patients and families to discuss hospice services and answer questionsMaintain accurate demographic, clinical, and payer informationStay up-to-date on compliance with local, state, and federal laws related to home health regulations, Medicare requirements, and care plan development techniques

Requirements:

To succeed in this role, you should have a graduate degree from an approved school of practical/vocational nursing and at least two years of experience as an LPN/LVN, preferably in oncology, long-term care, hospice, or home health. Three to five years of experience is preferred.
