Responsibilities- Develop and implement individualized patient care plans to meet the unique needs of each patient.
- Monitor and record patient condition, communicating effectively with healthcare teams to coordinate care and ensure seamless transitions.
- Educate patients and families on infusion services, medication management, and self-care techniques to promote optimal health outcomes.
- Collaborate with interdisciplinary teams to ensure comprehensive care, including patient transfer, discharge, and referral processes.
- Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program.
- State licensure as a Registered Nurse (RN).
- 6 months of nursing (RN) experience.
- BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date.
