Key Responsibilities- Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Conduct thorough assessments, identify patient needs, and develop effective care plans.
- Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Document patient progress, maintain accurate records, and communicate with the RN Supervisor as needed.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
