Key Responsibilities:- Provide Comprehensive Primary Care: Deliver direct primary care and preventative services to assigned children, adolescents, and adult patients.
- Develop Therapeutic Plans: Develop therapeutic plans of care for prevention, management, and maintenance of good health for patients.
- Health Education and Counseling: Provide health education and counseling to patients.
- Acute and Chronic Illness Care: Provide care for acute and chronic illnesses and urgent care services, including assessment, diagnosis, treatment, and monitoring of illness.
- Collaboration and Teamwork: Work closely with multidisciplinary teams and instruct and assist other team members in management of patient care.
- Practice Standards and Guidelines: Follow established practice standards and current evidence-based clinical practices and guidelines.
- Prenatal and Postnatal Care: Provide care related to prenatal and postnatal services and care for uncomplicated pregnancies, in conjunction with physician partners, as delineated and approved for in request for clinical privileges.
- Family Planning Counseling: Provide family planning counseling and services.
- Consultation and Referral: Consult with physician partners and refer patients with more complex/dynamic problems to subspecialty care, as appropriate.
- Electronic Health Records: Maintain and update Electronic Health Records (EHR) in a systematic format and complete required documentation within the specified amount of time expected by the Clinical Director.
- Confidentiality and HIPAA: Ensure that patient records are kept confidential consistent with Health Center policies and procedures, Joint Commission, and HIPAA standards. Monitor HIPAA compliance within the work environment.
- Training and Meetings: Attend trainings, weekly Provider and Team meetings, and collaborate on practice, administrative, and quality improvement projects, as assigned by Clinical Director.
- Clinical Supervision and Mentoring: Provide clinical supervision and mentoring to Nurse Practitioner, Medical, and Physician Assistant students by mutual agreement with the Clinical Director.
- Flexible Schedule: May work a fluctuating schedule, including days, evenings, weekend shifts, and holidays according to schedule approved by Clinical Director.
- Teamwork and Diversity: Support a work environment that is responsible and sensitive to the needs of a diverse staff and clients. Contribute to the professional team, supporting our Mission Statement.
- Education: Master's Degree as a Family Nurse Practitioner.
- Licensure: Active, Unrestricted Nurse Practitioner License in Massachusetts.
- Experience: 3+ years of experience as a Nurse Practitioner in a related field to Primary Care.
- COVID-19 and Flu Vaccine: The Health Center requires all employees to have the most recent COVID-19 Booster and the yearly Flu Vaccine.
- Salary: $105,000-$115,000/yr
- Loan Repayment Program: Qualifies for the HRSA Loan Repayment Program
- Health Insurance: Health Insurance plans starting on day-1 of employment. Employer pays 80% of premiums.
