Senior RN Case Manager Job Summary

The Southcentral Foundation (SCF) Senior RN Case Manager plays a vital role in delivering high-quality, patient-centered care to our customer-owners. As a seasoned RN Case Manager, you will be responsible for coordinating care, managing patient flow, and promoting cost-effective health outcomes within our established standards for case management practice and Advance Access principles.

Key Responsibilities:
  • Coordinate patient care, ensuring timely and beneficial outcomes
  • Develop and implement individualized care plans, taking into account patient needs and preferences
  • Mentor and coach staff on the Nuka System of Care, promoting a culture of excellence and teamwork
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes
Requirements:
  • Graduate of an accredited school of nursing
  • Licensed in the State of Alaska as a Registered Nurse
  • Basic Life Support (BLS) certification, with additional certifications as required for specialty
  • Demonstrated proficiency as a RN Case Manager at SCF or two (2) years RN supervisory or management experience
Employee Health Requirements:

Jobs designated as a Health Care Personnel (HCP) position require documentation of the following immunizations prior to your first day of employment: MMR (Measles, Mumps and Rubella), Varicella (Chicken Pox), Hepatitis B, Influenza, and T-dap (Tetanus - Diphtheria - Pertussis).

As an additional condition of employment, the successful applicant must be able to comply with the applicable standards of SCF's Background Check Policy, including the requirements imposed by the Alaska Barrier Crimes/Conditions and Background Checks regulations, the Indian Child Protection and Family Violence Prevention Act, the Crime Control Act of 1990, and the mandatory and permissive exclusion provisions of the Social Security Act.
