The Care Connections Nurse Practitioner plays a vital role in delivering high-quality, patient-centered care in the comfort of patients' homes, communities, and nursing facilities. As a Family Nurse Practitioner, you will be responsible for providing comprehensive medical assessments, ordering diagnostic tests, and developing treatment plans to address the unique needs of our patients.
Key Responsibilities- Provide general medical care and care coordination to patients of all ages, including adults, women's health, pediatric, and geriatric populations.
- Conduct comprehensive evaluations, including history and physical exams, to identify gaps in care and prevent future health issues.
- Address chronic and acute primary care complaints, and determine medical urgency to ensure timely interventions.
- Develop and implement medical plans of care, including ordering diagnostic tests and procedures as needed.
- Provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization.
- Participate in community-based 'Pop Up Clinics' to build relationships with the community and address health care gaps.
- Drive up to 120 miles a day to various locations within the assigned region, with occasional overnight hotel stays for Extended Mileage Special Projects.
- Master's degree in family health from an accredited nursing program.
- Advanced computer skills, including proficiency in Word, Excel, and Electronic Medical Record systems.
- Active and unrestricted national certification from a recognized organization, such as the American Academy of Nurse Practitioners or American Nurses Credentialing Center.
- Current state-issued license to practice as a Family Nurse Practitioner.
- Current Basic Life Support for Healthcare Professional certification.
- Unrestricted driver's license.
