We are seeking a skilled Physician to join our team at CenterWell Senior Primary Care. As a Physician, you will play a vital role in providing proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
Key Responsibilities- Provide high-quality patient care, focusing on personalized experiences and addressing the factors that impact patient well-being.
- Work collaboratively with integrated care teams, including physicians, nurses, and behavioral health specialists, to deliver compassionate and effective care.
- Develop and implement treatment plans, taking into account the physical, emotional, and social needs of patients.
- Stay up-to-date with the latest medical research and best practices, applying this knowledge to improve patient outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Bachelor's degree in a relevant field.
- 5 or more years of technical experience in a healthcare setting.
- Licensure requirements of the state of jurisdiction.
- Graduate of an accredited MD or DO program from an accredited university.
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine (preferred).
