- Assists patients with personal care tasks as outlined in their care plan, including hygiene, mobility, and elimination. If a task cannot be performed, it must be reported to the RN or Director of Clinical Services without delay.
- Ensures a safe environment for patients by using equipment correctly and safely (e.g., footstools, side rails, oxygen).
- Adheres to Universal Infection Control Precautions in all interactions with patients.
- Supports patients and families with home maintenance tasks as specified in the care plan, including light cleaning and meal preparation.
- Monitors and records essential health metrics such as fluid and nutrition intake, temperature, pulse, respiration, skin condition, and any changes in health status.
- Practices safe techniques for transferring, ambulating, and positioning patients.
- Recognizes when intervention from other home health team members is necessary and communicates this to the RN or Director of Clinical Services.
- Respects the autonomy and confidentiality rights of all patients and their families.
- Documents care provided and patient/family responses accurately and submits this documentation promptly.
- Operates within the scope of training and experience.
- Ensures compliance with local, state, and federal regulations, Medicare guidelines, and established agency policies.
- Manages a caseload effectively while delivering care in a cost-efficient manner.
- Consistently meets or surpasses the service standards set by 365 Health Services.
- Conducts all professional activities ethically and with integrity.
- Utilizes supplies and equipment in an effective and efficient manner.
- Completes a minimum of 12 hours of in-service training annually.
- Maintains consistent attendance.
- Adheres to the Code of Conduct established by 365 Health Services.
- Interacts positively with all staff, supporting the mission and vision of 365 Health Services.
- Complies with the agency's general orientation and policies, including confidentiality and HIPAA regulations.
