About the Role

Premise Health is seeking a skilled and compassionate Licensed Practical Nurse to join our team at the Key Health & Wellness Center. As a key member of our healthcare team, you will provide high-quality patient care and services to our clients in a fast-paced and dynamic environment.

Key Responsibilities
  • Assess and Evaluate Patient Needs: Conduct thorough interviews with patients to determine their medical problems and conditions, and document this information in their charts for physician review.
  • Provide Medical Treatment and Personal Care: Assist physicians with patient examinations, obtain and record vital signs and weight, and administer prescribed medication as needed.
  • Document Patient Services: Accurately document all services performed on patients, including laboratory tests and results.
  • Maintain a Clean and Safe Environment: Sterilize equipment and supplies, clean patient rooms, and maintain a safe and healthy environment for patients and staff.
  • Communicate Effectively: Respond to patient inquiries, follow up with patients who miss appointments, and coordinate referrals for patients through insurance and other physician offices.
Requirements
  • Education and Licensure: Graduate of a National League for Nursing accredited program, Associate's degree (A.S.) and/or equivalent from a two-year college or technical school, and licensed as a Practical or Vocational Nurse.
  • Experience: At least three years of clinical experience as a LPN/LVN in an ambulatory care, occupational health, or emergency department setting.
  • Certifications: Current hands-on Certification in Basic Life Support is required, Advanced Cardiac Life Support may also be required based on scope of contract services.
Preferred Qualifications
  • Knowledge and Experience: Knowledge about workplace health and safety concepts and OSHA regulations, strong computer skills, and knowledge and experience with Electronic Medical Records.
