Job Summary:

We are seeking a skilled RN Case Manager to join our team at Loyola Medicine. As a key member of our multidisciplinary team, you will work closely with physicians, staff, and payers to ensure patient progress along the continuum of care in an efficient and cost-effective manner.

Key Responsibilities:

  • Conduct patient assessments to identify complex clinical discharge needs and develop individualized plans of care.
  • Collaborate with the multidisciplinary team to ensure patient goals are set and negotiated healthcare resources are utilized effectively.
  • Provide patient education on available options of care throughout the continuum, ensuring safe and effective discharge.
  • Facilitate daily multidisciplinary rounds to identify and address care coordination barriers.

Requirements:

  • Minimum 3-5 years of previous job-related experience
