Key Responsibilities- Provide comprehensive primary care services to patients, including diagnosis, treatment, and coordination of care
- Develop and implement individualized treatment plans, taking into account patients' unique needs and circumstances
- Maintain accurate and timely documentation of patient records, ensuring compliance with regulatory requirements
- Collaborate with other healthcare professionals to ensure seamless transitions of care and optimal patient outcomes
- Doctor of Osteopathic Medicine (D.O.) or Doctor of Medicine (M.D.) degree required
- Completed residency in primary care specialty and Board Certification is required
- Minimum two (2) years of working experience in the practice of primary care medicine in an outpatient setting preferred
- Current licensure in the State of Illinois, DEA, Controlled Substance and maintain CME requirements
