We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team at CHS Recruiting. In this role, you will work collaboratively with a full primary care team to deliver high-quality patient care to our SNF residents.

Key Responsibilities and Duties

  • Manage a panel of SNF residents and provide high-quality patient care
  • Conduct comprehensive assessments and develop individualized plans of care
  • Communicate with family members/caregivers and ensure the care team understands the patient's goals and needs of care
  • Collaborate with physicians, care coordinators, facility staff, and other healthcare professionals
  • Perform direct care management support

Qualifications and Requirements

  • Active Michigan NP or PA license
  • Active DEA
  • Active Medicare credentials
  • 1+ year advanced practice experience
  • 1+ year geriatric care experience
  • Previous SNF or related experience
