Responsibilities- Conduct comprehensive assessments of assigned patients, documenting findings and developing individualized care plans.
- Support patients and families in meeting their spiritual, cultural, and educational needs, identifying learning levels and readiness for education.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Implement nursing interventions based on established care plans, prioritizing patient needs and modifying plans as necessary.
- Collaborate with interdisciplinary teams to develop and implement discharge plans, making referrals as needed.
- Provide safe medication administration and evaluate patient responses to nursing interventions, adjusting care plans accordingly.
- Lead and direct patient care or support techs, maintaining accountability for assigned patients.
- Demonstrate effective interpersonal skills, resolving conflicts and assuming charge nurse/preceptor roles as assigned.
- Current Registered Nursing license issued by the state of Illinois.
- Graduation from an accredited nursing program.
- BLS certification through the American Heart Association.
- PREFERRED: Bachelor of Science in Nursing, National Certification in area of specialty.
