Job Summary:

We are seeking a skilled and compassionate Home Care Nurse Aide to join our team at MyMichigan Health. As a Home Care Nurse Aide, you will provide high-quality patient care and support to individuals in their homes, promoting independence and dignity.

Responsibilities:

  • Provide hands-on personal care and assist patients with daily activities, such as bathing, dressing, and grooming.
  • Follow patient care plans and document progress, ensuring accurate and timely reporting.
  • Assist patients with ambulation, exercises, and other activities as needed.
  • Comply with infection prevention and control policies and procedures.
  • Develop and maintain positive relationships with patients and families, promoting a safe and secure environment.

Requirements:

  • High school diploma or equivalent required.
  • Valid certification as a Home Health Aide or equivalent required.
  • Basic Life Support (BLS) certification required within 1 month of hire.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.

Working Conditions:

Exposure to stressful situations, including those involving public contact, as well as, trauma, grief, and death.

Able to wear personal protective equipment, including latex materials or appropriate substitutes.

Must be able to move freely about the facility with or without an assisted device and perform the functions of the job as outlined in the job description.

Overall vision and hearing is necessary with or without assisted device(s).

Frequently required to sit/stand/walk for long periods of time.

May require frequent postural changes, such as stooping, kneeling, or crouching.

Some exposure to blood-borne pathogens and other potentially infectious materials.

Must follow MyMichigan Health blood-borne pathogen and TB testing as required.

Ability to handle multiple tasks, get along with others, work independently, and have regular and predictable attendance.

Overall dexterity is required, including handling, reaching, grasping, fingering, and feeling.

May require repetition of these movements on a regular to frequent basis.

Physical Demand Level:

Medium.

Must be able to occasionally (0-33% of the workday) lift or carry 21-50 lbs., frequently (34-66% of the workday) 20 lbs., and constantly (67-100% of the workday) 10 lbs.
