**Job Summary:**

We are seeking an experienced Advanced Practice Provider to join our team at Anthony Jordan Health Corp. In this role, you will provide comprehensive care and education to patients and families, working under the guidance of the Chief Medical Officer.

 

**About Us:

Anthony Jordan Health Corp is a Federally Qualified Health Center with Level III Patient-Centered Medical Home designation. We offer a network of outpatient primary care offices with providers who follow a panel of patients.

 

**Responsibilities:

In this role, you will be responsible for ensuring that high-quality patient-centered care is delivered. You will work closely with other medical professionals to coordinate services for high-risk, multi-problem families and assist families in identifying community resources to reduce barriers to access care.

 

**Requirements:

To be successful in this role, you must have completed an advanced educational program registered by the New York State Education Department or one determined to be equivalent by the New York State Education Department. You must also possess a current NYS license and be certified or eligible for certification in a specialty that includes the performance of primary health care services.

 

**Salary and Benefits:

We offer a competitive salary range of $46,800 - $52,000 per year for part-time positions and $93,600 - $104,000 per year for full-time positions. Additionally, we offer full benefits, including medical, dental, vision, life insurance, and a 403b retirement plan.
