Job SummaryWe are looking for a compassionate and experienced Registered Nurse to provide high-quality care to senior adults and individuals in need. The ideal candidate will have a strong background in nursing, with a focus on community health education and experience.
Responsibilities- Conduct initial assessments of patients and families to determine home care needs.
- Provide comprehensive physical assessments and histories of current and previous illnesses.
- Regularly re-evaluate patient nursing needs and make necessary revisions to the care plan.
- Develop and implement individualized care plans that address patient goals and needs.
- Administer medications and treatments as prescribed by physicians.
- Counsel patients and families on meeting nursing and related needs.
- Provide health education and instructions to patients as appropriate.
- Identify discharge planning needs and implement before discharge.
- California Registered Nurse License or California Vocational Nursing License.
- National Provider BLS - American Heart Association certification.
- 1-year professional nursing experience.
- IV and central lines experience.
- Experience with wound care, including negative pressure devices.
- Home health field experience.
