West Hartford, United States Optum Full time

Optum, a part of the UnitedHealth Group family of businesses, is revolutionizing healthcare by integrating industry-leading solutions to create a holistic care model that addresses an individual's physical, mental, and social needs.

We are seeking highly skilled Advanced Care Specialists to join our Senior Community Care (SCC) team. As a member of this team, you will work closely with a group of professionals to provide care to patients at home, in nursing homes, assisted living facilities, or senior housing.
