Social Worker

This role will involve collaborating with Care Managers to develop and implement care plans that address the complex health and social challenges faced by our Dual Eligible Special Needs Plan (DSNP) members. You'll work closely with members, providers, and community organizations to address the full continuum of their healthcare and social determinant needs.
Key responsibilities will include:
- Participating in the care planning process to assess, goal-set, and establish interventions related to goals
- Conducting telephonic and face-to-face interactions with members, including visits to their homes, in the community, and/or provider locations
- Developing and carrying out strategies to coordinate and integrate post-acute and long-term care services to prevent exacerbations and/or placement of members in custodial care
- Applying social work concepts, principles, and strategies in addressing the social determinants of health needs in members' individualized care plans
- Serving as a member advocate to ensure they receive all necessary care allowed under their benefit plan and as available through Medicaid benefits and/or other community resources
Requirements include:
- Minimum of 2+ years' experience in medical social work or case management
- Active and unrestricted LCSW or LMSW in OH
- Ability to travel 50-75% within Cincinnati
- Master's degree in Social Work
