Hospice Social Worker

(405900) Medical Services of America Hospice of the Lowcountry seeks a dedicated Full-Time Hospice Social Worker / MSW to provide compassionate care to patients and families in our service areas.

We are looking for an experienced professional to join our interdisciplinary team, providing social work services in accordance with the physician's plan of treatment. This role involves working closely with the healthcare team to assess patient needs, develop personalized care plans, and coordinate support services.

The ideal candidate will have a Master's or Bachelor's degree in Social Work from a Council of Social Work Education-accredited program and be licensed to practice as a social worker in South Carolina. A minimum of one year of Hospice experience is preferred.

• Conduct comprehensive assessments of patients' physical, emotional, and social needs
• Develop and implement individualized care plans in collaboration with the healthcare team
• Provide bereavement counseling and support to patients and their loved ones
• Collaborate with community resources to ensure seamless care coordination

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience. Additional benefits include:
